Comments:
An unannounced monitoring inspection was conducted beginning at 1:00 p.m. to 4:00 p.m. with the assistant director and director. There were 29 children in care ranging in age from 7 months to 5 years of age under the care and supervision 3 staff. The infants were observed sleeping, being held and being put down for nap. Toddlers, two year olds, three year olds, four year olds and five year olds were observed during nap. The outdoor play area was observed. Children woke during the inspection and had bathroom time and play time. Four records and six children's records were reviewed. The center does not administer medications: Medication Administration Training (MAT) is required to administer medication. Standard #: 22VAC40-185-160-C
Description: Based on review of staff records, the center failed to obtain Tuberculosis (TB) screening or test every two years. Evidence: Staff #3's most recent TB test was dated 3/24/2017.

Plan of Correction: The staff will schedule a TB test as soon as possible.

Standard #: 22VAC40-185-270-A
Description: Based on observation, not all areas and equipment of the center were maintained in safe, operable condition. Evidence: In the infant room, there is a recalled purple Bumbo brand seat. In the multipurpose room, there is a 32 inch flat screen television that is on a shelf approximately four feet high that is not anchored that poses a crush hazard. In the downstairs preschool classroom, the bathroom is still labeled "out of order". On the playground, there is a little tikes brand car that has two bolts exposed that create an impalement hazard, scratch hazard and entanglement hazard.

Plan of Correction: The Bumbo seat will be removed and the safety straps have been ordered during the inspection. The tv will be moved immediately. There was delay with remodeling and a discussion is in progress with the landlord. The little tikes car was removed from the playground.
