Technical Assistance:
Technical assistance today included record keeping, physical environment and staff training.

Comments:
An unannounced monitoring inspection took place today from 10 am - 12:45 pm. There were 117 children present. All classes were within ratio. I observed indoor and outdoor activities including a science "lab" where children were predicting what would float; reading stories and involved in dramatic play. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation during today's inspection, some areas were found to not be maintained in a safe condition. Evidence: Indoors: There were some cracked floor tiles in the Discovery Room which could pose a tripping hazard. Outdoors: A wooden fence surrounding an air conditioning/heat pump unit was broken (collapsingn on one side) with exposed nails accessible. There were areas of sharp siding edges accessible at child height in the siding and at the bottom of the drainiage pipe. There are chipping paint on the yellow slide, and evidence of rust. The shed had an area on the bottom board that was broken and splintery, leaving an opening for finger entrapment. There were also two exposed nails.

Plan of Correction: We hammered nails in and contacted facilities to come. Until then, we will secure the area where the fence and shed are hazardous.

Standard #: 22VAC40-185-420-E-1
Description: Based on a review of infant records today, not all required information was given to parents. Evidence: On 6/3 and 6/4/19, the amount of time infants who cannot yet turn over on their own were placed in "tummy time" was not documented for four children.

Plan of Correction: We will retrain the staff on the documentation of "tummy time".

Standard #: 22VAC40-185-510-N
Description: Based on a review of medication records, one child's medication was on site after no longer being authorized. Evicence: The short term medication for Child # 1 was present, although the authorization expired 3/18.

Plan of Correction: The child attended during spring break and will return in the summer. The parent will be contacted and the medication will be returned.

Standard #: 22VAC40-185-550-D
Description: Based on a review of records, monthly emergency evacuations were not consistently practiced and documented. Evidence: No drill was documented for March, April and May.

Plan of Correction: We will do make up drills.
