Comments:
The licensing inspector conducted an unannounced renewal inspection on June 5, 2019 to determine the center's compliance with licensing standards. The inspection was initiated at 9:35 am and completed at approximately 2:40pm. The center's director, assistant director, and executive director were present and assisted the inspector throughout the inspection. The census for today's inspection consisted of 88 children in the direct care of 18 staff. Upon arrival, the children were engaged in a variety of activities. The inspector observed and inspected each classroom and completed formal observations throughout. The children in the Preschool IA room were observed during free-play, dancing, interacting with peer and staff. The children in the Pre-K A were observed during a teacher-led art activity called "Sea Animal Matching." The theme of the week is Oceans. The children were also observed as they practiced writing their name. The children in the Preschool IIB room were observed as they prepared to go out to the playground. The staff were observed doing head-counts as they exited the classrooms. The children in the Twos A room were observed during "choice time." The children in the Preschool IIA room were observed during an art activity and free-play. The children in the Toddler B room were observed drinking water at the table and interacting with peer and staff. Diapering and hand-washing procedures were also observed in these rooms. The children in the Infant A room were observed interacting with staff, sleeping, and being fed. The children in the Infant B room were observed playing on the carpet, interacting with staff, and having a story read to them. The playgrounds and three vehicles were inspected. The center had the following posted: license, daily schedule, emergency numbers, evacuation maps, menu and various parent information. The center's first aid kit and non-emergency supplies were inspected. Three medications and medication authorizations were reviewed. Eight children's records and eight staff's records were reviewed. The most recent fire inspection was documented on 12/14/2018, the most recent health inspection was documented on 05/31/2019. The most recent evacuation drill was documented on 05/31/2019, the most recent shelter-in-place drill was documented on 04/19/2019, and a lockdown drill on 09/17/2018. Violations:
Standard #: 22VAC40-185-500-B
Description: Based on observation and interview on 06/05/2019, the center did not ensure diapers are disposed of in a system that is not hand operated. EVIDENCE: 1) During the inspection, the licensing inspector observed the trash can used for diapering in the Twos A room is hand operated. The licensing inspector observed a staff member using her hands to open the diapering trash can to dispose of a soiled diaper. 2) In the Infant B room, the licensing inspector observed the trash can used for diapering is inoperable. Disposable diapers shall be disposed in a leakproof or plastic-lined storage system that is either foot operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.

Plan of Correction: Corrected on site. Per the Center: We will make sure all trash cans in all diaper changing classrooms are operating properly by checking each trash can weeekly.

Standard #: 22VAC40-185-540-C
Description: Based on observation on 06/05/2019, the center did not ensure to have all required first aid supplies on the vehicles used to transport children. Evidence: Three of the center's vehicles used to transport children were inspected. The following required items were missing from the first aid kits: 1) Bus #1did not have a second triangular bandage. 2) Bus #2 did not have a second triangular bandage. 3) Bus #3 did not have a working thermometer, scissors, and a second triangular bandage.

Plan of Correction: Corrected on site. Per the Center: We will make sure to check the bus [sic] first aid kits weekly to ensure we are in compliance.

Standard #: 22VAC40-191-60-B
Description: Based on a review of eight staff records on 06/05/2019, the center did not ensure that one staff had a completed sworn statement or affirmation prior to the beginning of employment. Evidence: 1) The record for Staff #1, whose hire date was 05/13/2019, contained a sworn statement that was dated 05/16/2019.

Plan of Correction: Per the Center: We will make sure orientation and orientation paperwork are completed on the same day.
