Comments:
The licensing inspector conducted an unannounced Subsidy Health and Safety Inspection on June 5, 2019 to review supplemental health and safety requirements. The inspection was initiated at 9:35 am and completed at approximately 2:40pm. The center's director, assistant director, and executive director were present and assisted the inspector throughout the inspection. The census for today's inspection consisted of 88 children in the direct care of 18 staff. Upon arrival, the children were engaged in a variety of activities. The inspector observed and inspected each classroom and completed formal observations throughout. The children in the Preschool IA room were observed during free-play, dancing, interacting with peer and staff. The children in the Pre-K A were observed during a teacher-led art activity called "Sea Animal Matching." The theme of the week is Oceans. The children were also observed as they practiced writing their name. The children in the Preschool IIB room were observed as they prepared to go out to the playground. The staff were observed doing head-counts as they exited the classrooms. The children in the Twos A room were observed during "choice time." The children in the Preschool IIA room were observed during an art activity and free-play. The children in the Toddler B room were observed drinking water at the table and interacting with peer and staff. Diapering and hand-washing procedures were also observed in these rooms. The children in the Infant A room were observed interacting with staff, sleeping, and being fed. The children in the Infant B room were observed playing on the carpet, interacting with staff, and having a story read to them. The playgrounds and three vehicles were inspected. The center had the following posted: license, daily schedule, emergency numbers, evacuation maps, menu and various parent information. The center's first aid kit and non-emergency supplies were inspected. Three medications and medication authorizations were reviewed. Eight children's records and eight staff's records were reviewed. The most recent fire inspection was documented on 12/14/2018, the most recent health inspection was documented on 05/31/2019. The most recent evacuation drill was documented on 05/31/2019, the most recent shelter-in-place drill was documented on 04/19/2019, and a lockdown drill on 09/17/2018. A licensing inspection was also conducted this date; the two inspections combined over all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice. Standard #: 22VAC40-665-540-A
Description: Based on a review of eight staff records on 06/05/2019, it was determined that the vendor did not ensure that two staff shall be evaluated by a health professional and be issued a statement that the individuals are determined to be free of communicable tuberculosis (TB) within the required time frame. Evidence: 1) The record for Staff #1, whose hire date was 05/13/2019, contained documentation of a TB screening dated 05/23/2019. 2) The record for Staff #2, whose hire date was 06/03/2019, contained documentation of a TB screening dated 06/04/2019. Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.

Plan of Correction: Per the Vendor: We will make sure all staff have TB and VA Preservice training completed before starting employment with us.

Description: Based on a review of records on 06/05/2019, the vendor did not ensure that two of eight staff records contained documentation of the required Virginia Preservice Training for Child Care Staff within 90 days of employment. Evidence: 1) The record for Staff #4, whose hire date was 01/28/2019, did not contain documentation of the required Virginia Preservice Training. 2) The record for Staff #6, whose hire date was 02/13/2019, did not contain documentation of the required Virginia Preservice Training.

Plan of Correction: Per the Vendor: We will make sure all staff have TB and VA Preservice training completed before starting employment with us.
