Comments:
A subsidy health and safety inspection was conducted with the director. Four allergy plans were reviewed today. The center is up to date with the 10 hours of pre-service training for the staff and their lock-down drill. Arrival time: 12 noon Departure time: 2: 15 pm

Violations:
Standard #: 22VAC40-665-520-B-11
Description: Based on a review of files and an interview with the director, it was determined that one child who has a diagnosed food allergy to milk did not have an allergy plan on site. Evidence: Child A has a diagnosed milk allergy but there was no allergy plan from the physician on site.

Plan of Correction: This is a parent preference and not a food allergy, currently child is not served whole milk at the school. Based on food from home note ( signed by physician) water will be given in place of milk. If child was accidently to ingest milk, no preventative medication or medical treatment is needed. Current course of action is to notify mother. We are currently reviewing information with parent and physician to clarify any other course of action needed.
