Technical Assistance:
Discussed bookshelves that are showing signs of peeling paint and non sleeping mats which are showing signs of excessive wear.

Comments:
An unannounced monitoring inspection was conducted today from 9:00am-10:15am. There were 27 children directly supervised by 4 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in being dropped off for care, playing outside, and transitioning to classrooms. Hand-washing procedures were also observed. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children. Evidence: A canister of aerosol air freshener was observed in an unlocked cabinet in the Caterpillar classroom.

Plan of Correction: The cabinet was locked during the inspection.

Standard #: 22VAC40-185-510-J
Description: Based on observation, medication was stored in an unlocked place. Evidence: An unlocked cabinet in the Caterpillar classroom contained: an epinephrine auto-injector pen, liquid antihistamine, liquid Ibuprofen, and hydrocortisone cream.

Plan of Correction: The cabinet was locked during the inspection.

Standard #: 22VAC40-185-510-N
Description: Based on a review of medication, medications were not picked up by the parent or disposed of within fourteen days of the medication authorization's expiration date. Evidence: Four medications for Child 1 were present in the facility on the date of inspection. The written authorizations on file for the medications had a documented end date of 4/9/2019.

Plan of Correction: We have made the parents aware of the expired documents and have already informed them to bring in updated forms. The family has been out of the country for several weeks and are currently working with their pediatrician to get us the new authorization forms.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review, one staff member did not have a central registry finding within 30 days of employment or volunteer service. Evidence: Staff 2's record did not contain the findings of a central registry search. (Date of Hire: 4/8/2019).

Plan of Correction: The request has been sent and the canceled check for the request was shown to the licensing inspector. We will follow up with the agency to determine the results of the search.
