Comments:
An unannounced renewal inspection was conducted on 06/06/2019 from 9:15 A.M.-11:45 A.M. There were eleven children present with three staff and one administrator present. Six children records, three staff records, one medication and required documentaion, over-the-counter skin products and required documentation, first aid kit and emergency supplies, evacuation drills, outdoor play area, one injury report, and required posted information were reviewed. Children were observed during free choice activities, transition times, diapering, handwashing, outdoor play, snack time, music, movement, and during parachute play. Violations:
Standard #: 22VAC40-185-500-B
Description: Based on observation, the center failed to ensure that a nonabsorbent surface for diapering was used. Evidence: 1. The diaper changing table pad, located in the classroom used for snack time and lunch, had two holes exposing the foam making it absorbent. 2. Staff verified that the diaper changing pad had two holes and the foam was exposed.

Plan of Correction: Administration will order another diaper changing pad to replace the one with holes.

Standard #: 22VAC40-185-510-G
Description: Based on a review of prescription medications, the center failed to obtain the prescription labels for one medication. Evidence: 1. Review of medications for Child 1M determined that the medication prescription label had not been obtained. 2. Staff verified that the medication was missing the prescription label.

Plan of Correction: Administration will contact the parents of the child to obtain the box with prescription label.

Standard #: 22VAC40-185-520-A
Description: Based on review of over-the-counter skin products, the facility did not follow procedures for administering over-the-counter skin products in accordance with the manufacturer's recommendations. Evidence: 1. A bottle of Honest Mineral Sunscreen that expired in April 2019 was in the bin of over-the-counter medications. 2. A can of Honest Mineral Bug Spray that expired in February 2018 was in the bin of over-the-counter medications. 3. Staff verified that the sunscreen and insect repellent were expired.

Plan of Correction: Staff immediately removed the sunscreen and insect repellent from the bin, and will return to the parent at pick-up. Administration will review with staff the requirements for over-the-counter skin products, and remind staff to check expiration dates on all products before accepting them from the parents.
