Comments:
The licensing inspectors conducted an unannounced complaint inspection in response to a complaint regarding supervision that was received by the licensing department on 04/30/2019. The inspection was initiated and completed on 05/22/2019. On 05/22/2019, the inspectors were on site from 3:40 PM to 4:50 PM. Interviews with staff were completed, observations were made in the center, and documentation was reviewed. The preponderance of the evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. See the violation notice for more details. Description: Based on observation and interviews, the center did not ensure that when children are regularly in ongoing mixed age groups, the staff-to-children ratio applicable to the youngest child in the group, was applied to the entire group. Evidence: 1. At approximately 3:40 PM, Staff #1 and Staff #2 were observed taking 15 children from the gym into a classroom. Once in the classroom, children continued to arrive from nearby elementary schools from approximately 4:05 PM until 4:15 PM. A total of 21 children were observed in the classroom, being supervised by Staff #1 and Staff #2, when the applicable staff to child ratio was 1:8. Staff #1 and Staff #2 stated the youngest child in the classroom was Child A (two years of age) and the oldest children in the classroom were up to 12 years of age. Staff #1 stated ?the children in the classroom were the wrap around Preschool Head Start program up to the school age children?. Staff #1 and Staff #2 stated on a regular basis there is a maximum average of 25 children in the group in the afternoon. Staff stated the wrap around Head Start program (three year olds) stay in the classroom with the older children. Staff #1 stated the three year old children through the 12 year old children were regularly mixed in the classroom every day from 2:00 PM when Head Start program ends until 6:00 PM. Staff #1 stated ?We keep the children divided into groups with lots of stuff to do; we keep the little kids away from the big kids?. Staff #1 stated ?three to four times a week a third staff member will be in the classroom but not every day?.
