An unannounced monitoring visit was done today June 4, 2019 beginning at 7:05 A.M. and ending at 9 AM.. There were 25 children in care today with three staff. The children were observed during arrival, table toys and departure for a field trip. The inspector reviewed five staff's and five children's records. One medication was reviewed. Discussion with director about training requirements, physicals, and religious exemptions .

Standard #: 22VAC40-185-60-A Description: Based on review of five records, the center failed to ensure that all records were complete. Evidence: 1. Child # 1 was missing the chronic physical problems. 2. Child # 3 was missing an address for one of the two require emergency contacts. Plan of Correction: The director has informed the parents of the missing information and will ensure all information is up to date.
