Comments:
An unannounced monitoring inspection was conducted on June 3, 2019 from approximately 9:30 AM to 12:30 PM to determine the facility's compliance with licensing standards. The staff to child ratios were observed as follows: Infants/Toddlers 2:7, 2 year olds 2:11, 3 year olds 2:11, 4 year olds 1:10, Pre-K 2:9. School age children were not present during the inspection. Lunch today consisted of ravioli with meat sauce, corn, pineapple, and milk. All areas used for child care were inspected. The facility has age appropriate materials and supplies. The following required information was posted; the License, emergency numbers, evacuation map/procedures, daily schedules, menu, and various parent information. The first aid kits and non-medical emergency supplies were inspected. The inspector interacted with staff and children in care. Five staff records and six children's records were reviewed. Medications and authorizations were reviewed. There are staff on site with current First Aid, CPR, MAT and DHO training. The most recent evacuation drill was documented on 5/3/19. The most recent shelter-in-place drill was documented on 9/17/18. I Please complete the "plan of correction" and "date to be corrected" for each violation cited on the violation notice and return it to the licensing office within 5 business days. Your plan of correction should contain 1) steps to correct the non-compliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Description: Based on observation, the center did not ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Evidence: The licensing inspector observed areas of cracked and chipped exterior building siding located in the area of the toddler playground.

Description: Based on observation, the center did not ensure that hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. The licensing inspector observed an unlocked spray bottle of air freshener in the two year old classroom's bathroom. 2. The licensing inspector observed a bottle of unlocked hand sanitizer in the three year old classroom.

Evidence: The licensing inspector observed the first aid kit in the center's van did not contain a thermometer. The first aid kit in the center's van did not contain an antiseptic cleansing solution or pads, a first aid instructional manual, nor a functional ice pack or cooling agent.

Evidence: During the inspection, staff were unable to provide a record of the center's shelter-in-place drills. The licensing inspector advised staff the records could be provided by close of business on 6/4/19 if located by the director. The licensing inspector did not receive documentation of the shelter-in-place drills.
