Comments:
A complaint was received involving sight and sound supervision. The complaint was determined to be valid and there was one violation resulting from this investigation.

Additionally, the center self reported a situation involving forbidden action with a staff being rough with a child and staff documenting that they fed an infant when they had not. There were two violations resulting from the self report situations.

Please provide your plan of correction to Licensing by May 21, 2019.

Description: Based on interviews and a review of documentation, it was determined that the center's staff failed to ensure a child's care, protection and guidance. Evidence: On 4/2/19, staff documented that they had fed child A ( 4 months of age) their bottle at 9:30 a.m. when in fact the staff did not feed the infant at that time. This was discovered by the parent at approximately 12: 40 pm when the parent arrived to pick up their child for the day and noticed that the infant had an extra full bottle left over for the day.

Plan of Correction: In reference to the incident, all infant staff was retrained on the proper procedures of documenting feedings for the infants. All staff was retrained and reminded that it is against licensing as well as CBA policy to pre-enter information for children. On the day of the incident, a new procedure was put into place for the verification of feedings and documenting them. When one staff is going to feed a child, they must verbally state what they are doing and enter the information at that time. As they enter the information, a second staff member must be a witness to this procedure. If the staff is a single, they will call the office for verification.

Description: Based on interviews, a review of a written statements and a video recording, it was determined that a staff left two children ( 2 years of age) alone and out of sight and sound supervision.
Evidence:
On 4/8/19 at 7:40 am staff A left child B and child C ( 2 years of age) alone in the Beginner B classroom while she retrieved a forgotten item in Beginner A classroom. A second staff entered the classroom and discovered the two children alone. The investigation revealed that the children were left alone for approximately one minute.

Plan of Correction: In reference to the incident, Staff A was immediately reprimanded surrounding her actions. Staff A received one on one training in the proper procedures to follow to prevent a sight and sound supervision issue. Prior training was provided on 2/18/19 and 3/12/19 as well as training after on 4/30/19 with all of the staff. Future actions is that staff will call the front office for assistance as needed

Description: Based on interviews, a review of a written statement and a video recording, it was determined that a staff roughly handled a toddler.
Evidence:
While sitting on the carpet during circle time at approximately 3:30 pm, staff B took child D's legs and with force pulled the child toward her which resulted in the 17 month old being dragged on the carpet. Immediately after this incident child D could be seen standing up on the video recording and staff A took the child by the torso and harshly placed the child back on the carpet in a sitting position.

Plan of Correction: Since the incident in January of 2019, staff have been retrained on the importance of proper handling of children and that they are all mandated reporters. During staff meetings, trainings have been done with staff regarding forbidden actions on 2/18/19, 3/12/19, and 4/30/19. Mandated reporter training was done on 1/29/19 with all current staff and has been implemented in with our new hire procedures. Training with an outside source, Paula Bellville from the Infant and Toddler Specialist Network, was done on 2/26/19 on positive interactions with children. In reference to the incident, staff has made progress as the incident was reported by another staff member that witnessed it. While this does not excuse the actions of Staff B, it shows the understanding of the other staff in the importance of speaking up. Future actions are to continue to be diligent in monthly trainings and observations in the classrooms. While observing classrooms in person is beneficial, the review of the cameras regularly will also be beneficial in monitoring staff to child interactions.
