An unannounced complaint inspection was initiated on May 29, 2019 and concluded on June 11, 2019 regarding allegations involving staffing/supervision and special care/precautions. Classroom observations, staff interviews and documentation review were conducted. Based on observations, interviews and review, there is not a preponderance of evidence to support the allegation therefore the complaint is deemed not valid. A violation was found that was not reported. Please see violation notice for further details.

Description: Based on review, facility did not ensure required information was documented when a child sustains an injury. Evidence: Review of incident/accident report completed on March 21, 2019 did not document the date parent was notified of the injury.

Plan of Correction: All staff were trained to ensure all incident reports are filled out completely to include dates, times and person responsible for assigning incident reports.
