Comments:
An announced inspection was conducted at the request of the licensee for a modification. The cafeteria and classroom #7 were measured and the physical plant, required supplies and postings were also reviewed. Areas of non-compliance are noted on the violation notice. The results of the inspection were discussed with the center director during the exit interview. The modification request will be reviewed with the Licensing Administrator for license issuance. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation and staff interviews, the licensee did not ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin. Evidence: In the cafeteria, on the window frame in the children's play area, there was a loose metal hook with a protruding bolt. The hook was approximately 2 inches long and was sharp.

Plan of Correction: The bolt and hook was removed.

Standard #: 22VAC40-185-280-B
Description: Based on observation and staff interviews, the licensee did not ensure hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. Evidence: In the cafeteria, the kitchen door was unlocked and there was one container of detergent, one container of cleaner and one container of disinfectant. All three containers were labeled "keep out of reach of children" .

Plan of Correction: Center director will make the door is locked.

Standard #: 22VAC40-185-500-B
Description: Based on observation and staff interviews, the licensee did not ensure a nonabsorbent surface for diapering or changing shall be used. Evidence: In room #7, the diaper pad had a small rip that had exposed foam. The diaper pad was no longer nonabsorbent.

Plan of Correction: The diaper pad was thrown away and another one will be used.
