Comments:
An unannounced renewal inspection was conducted on Monday, June 10, 2019 to determine the center's compliance with licensing standards and to follow up on previous violations. The inspector arrived at 10:10am and concluded the inspection at 12:40pm. The census for today?s inspection consisted of 71 children in the direct care of 16 staff. The center?s director assisted the inspector throughout this inspection. Upon the inspector's arrival, the children were engaged in a variety of activities. The inspector observed and inspected each classroom and completed formal observations throughout. The children in the Infant 1 and Infant 2 rooms were observed interacting with staff, crawling, and playing with various age appropriate toys. Diapering and hand-washing procedures were also observed in these rooms. The children in the Toddler 1 and Toddler 2 were observed during free-play and during group activities. In the Early Preschool 1 room, the children were observed in two separate groups while they interacted with peers and staff. A group of children was observed playing and learning about different instruments and another "digging for treasures" at a sensory table. The Early Preschool 2 class was observed in the playground and during transitions. The staff were observed doing name-to-face checks as the children moved from one location to the next one. The children in the Preschool 1 and Preschool 2 rooms were observed playing in centers. In the Kindergarten Prep I room, the children were observed playing Bingo and learning about shapes and colors. There are currently no school age children enrolled in the program. The areas where children receive care, inside and outside, were inspected and found to be in compliance. The center had the following posted: license, daily schedule, emergency numbers, evacuation maps, menu and various parent information. Seven children's records and seven staff's records were reviewed during today's inspection. Emergency medications and authorizations were reviewed. There are several MAT certified staff on site, as well as staff with Daily Health Observation training. The menu was posted and today's lunch consisted of milk, cheese quesadillas, mandarin oranges, pico de gallo, and black beans.

The most recent health inspection was documented on 01/16/2019, the most recent fire inspection was documented on 05/09/2019. The last evacuation drill was held on 05/15/2019 and the most recently documented shelter-in-place drill was held on 09/17/2018.

Please complete the `plan of correction? and `date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from the date of receipt. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standards, 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Violations:
Standard #: 22VAC40-185-160-C
Description: Based on a review of seven staff records on 06/10/2019, the center did not obtain an updated tuberculosis (TB) test or screening every two years for one out of seven staff records reviewed. EVIDENCE: 1) The TB test for Staff #1, hired on 06/05/2017, was dated 04/21/2017 and expired on 04/21/2019. Staff shall resubmit TB test results every 2 years from the date of the first initial screening or testing, or more frequently as if recommended by a licensed physician or the local health department.

Plan of Correction: Per the Center: "Staff #1 is a summer sub and was away at school still on the date of the expiration of the TB test. Upon her return to work for us in June she was sent to obtain a new TB test, following the 30 day rule of obtaining a TB test within 30 days of a staff member starting."

Standard #: 22VAC40-185-280-B
Description: Based on observation at the center on 06/10/2019, the center did not ensure to keep hazardous substances in a locked place using a safe locking method that prevents access by children. Evidence: 1) The licensing inspector observed a bottle of disinfectant spray on a shelf in the Early Preschool I's bathroom. The disinfectant spray was accessible to the children in care. Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

Plan of Correction: Per the Center: "Corrected on site. The bottle was on a shelf that was not within reach of the children. Administrative staff will monitor this daily and will ensure that the teachers are reminded to always lock up the cleaning solution when not in use."

Standard #: 22VAC40-185-290-3
Description: Based on observation at the center on 06/10/2019, the center did not ensure all electrical outlets have protective covers. EVIDENCE: 1) The licensing inspector observed two electrical outlets in the Toddler 2 room without protective covers. In areas used by children of preschool age or younger electrical outlets shall have protective covers.

Plan of Correction: Per the Center: "Corrected on site. The outlet is already child safe and does not require an outlet cover."
