63.2(17) License & Registration Procedures

Comments:
An unannounced monitoring inspection was conducted from 10:00 a.m. - 12:30 p.m. There were 136 children in care being supervised by 22 staff members. The children were observed participating in circle time activities, table top activities, outdoor play, and eating snack. All meals are provided from home. Ten children's records were reviewed and in compliance. Ten staff records were reviewed. Thirteen medications were reviewed. The most recent emergency escape drill was conducted 05/07/2019 and the most recent shelter in place drill was conducted 04/08/2019. Violations:
Standard #: 22VAC40-185-330-B
Description: Based on observation of the playground it was determined that not all climbing equipment had appropriate resilient surfacing to create a fall zone free of hazardous obstacles. Evidence: There was less than 1/2 of mulch present and some areas where the ground was present in the fall zone of the rock climbing structure. This piece of equipment would require a minimum of 6" of resilient surfacing in the entire fall zone of the structure.

Plan of Correction: This piece of equipment was placed off limits during the inspection. Remains closed until timbers and mulch are completed.

Standard #: 22VAC40-185-500-B
Description: Based on classroom observation it was determined that not all diapering procedures were followed. Evidence: 1. The changing pads in the young toddler room and infant room had multiple rips on the front of the changing pad making the changing pad absorbent. 2. A fabric cover was observed on the changing pad in the two year old room making the changing pad absorbent.

Plan of Correction: Pads were replaced and cloth discarded.

Standard #: 22VAC40-185-510-G
Description: Based on review of medication it was determined that not all procedures for administering prescription medications were followed. Evidence: An emergency medication for child A was observed without a prescription label and not maintained in the original labeled, container.

Plan of Correction: Parent provided paperwork from physician.

Standard #: 22VAC40-185-520-C
Description: Based on observation of the toddler room (Room #8) it was determined that not all procedures for administering diaper ointment were followed. Evidence: One tube of diaper ointment was observed in the bottom of an unlocked drawer of the changing table which was accessible to children.

Plan of Correction: The administrator removed the diaper ointment during the inspection. Reminded staff ointment needs to be in box out of reach and inaccessible.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on review of ten staff records it was determined that not all records contained an out of state central registry check for staff who have lived outside of Virginia within the last five years. Evidence: There was no documentation of an out of state central registry check for staff A who's sworn statement form (dated: 08/27/2018) indicated living outside of Virginia within the last five years. This is a repeat violation.

Plan of Correction: Resend, request, and copy paperwork before mailing.
