Comments:
An unannounced monitoring and complaint investigation was conducted on April 25 2019 between 4:00pm and 5:45pm. Based upon observations and interviews, the preponderance of evidence regarding the allegation of supervision is deemed not valid. There were 23 children in the direct care of 2 staff members. Children were observed playing the gymnasium, reading, completing homework, and eating snack. The program is conducted in the elementary school cafeteria and gymnasium. The areas were clean and organized. Areas of non-compliance are listed on the violation notice. Description: Based on observation and staff interview, staff do not have documentation that a staff member on site has satisfactorily completed a training program to administer medication. Evidence: A staff member stated that she has a Medication Administration Training (MAT) certificate, but was unable to locate it during the inspection.

Description: Based on observation, the center could not provide documentation that at least one staff member on duty has obtained, within the last three years, instruction in preforming the daily health observation of children. Evidence: There was not documentation that a staff on site had the training.
