Technical Assistance:
Technical assistance was provided regarding children's records and documentation of parents' work information.

Comments:
An unannounced monitoring inspection was conducted today from 9:30am-12:00pm. There were 89 children between ages 4 months and 10 years old directly supervised by 13 staff and all classrooms were in ratio. The physical plant, playground, programming, menu, 5 staff records, 9 children?s records, 5 medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed playing on the playground, building with blocks, doing puzzles, eating lunch, reading, making crafts, and playing games. Lunch served today was a stacked enchilada bake, pinto and refried beans, whole grain flour tortilla, corn and salsa, fresh green salad, and milk. Infant care including feeding was also observed. Positive interactions between children and staff were observed during this inspection. There was an abundant supply of age appropriate materials for the children and the center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-60-A
Description: Based on review of 9 children's records, 1 child's record did not contain all required information. Evidence: 1. Child #8 did not have complete addresses listed for 2 of 2 required emergency contacts. 2. Child #8's record did not contain documentation that an annual review of emergency contact information has been completed within the last year. The last review of emergency contact information was dated 3/1/17 by the child's parent.

Plan of Correction: We will review all children's files to ensure all required information is included. We will obtain missing required information from parents.

Standard #: 22VAC40-185-500-B
Description: Based on observation, a foot operated disposal system, or another method that prevents the staff's hand or soiled diaper from touching the exterior of the trash can, was not available for disposal of soiled diapers. Evidence: 1. In the Discovery Preschool classroom, there are several children in diapers or pull-ups. The foot operated trash can located next to the changing table was broken as the lid could not be reattached to the base making it no longer foot operated. 2. A second trash can was located in the Discovery Preschool classroom's bathroom and this trash can's foot pedal was broken off and the lid was not attached making it no longer able to be foot operated.

Plan of Correction: A trash can for the diapering area has already been ordered. I will place it in the classroom when it arrives. I will also order another replacement foot operated trash can for the bathroom.
