Discussion held regarding chap-stick and other topical preparations.

Comments:
Licensing Inspectors arrived at 10:10 AM and departed at 12:30 PM. At the time of the inspection, there were 119 children (ages infant through school age) in care with 16 staff. The children were observed outdoor play, making Father's Day gifts, circle time and other morning activities. The sample size was 13 staff and 12 children's records. Medication, accident reports and center documents were all reviewed.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on a review of 12 children's records, it was determined that the facility did not ensure that, before attending the center, they obtained documentation of the child's immunizations or the "Certificate of Religious Exemption". Evidence: The record for child #5 indicated on the immunization form that the child did not have immunizations for religious reasons. The record did not include a copy of the Certification of Religious Exemption.

Plan of Correction: Center management will have the parent complete the Certificate of Religious Exemption and place it in the record.

Standard #: 22VAC40-185-160-C
Description: Based on review of 13 staff records, it was determined that the facility did not ensure that at least every two years from the date of the last screening, staff submit the results of a follow-up tuberculosis (Tb) screening. Evidence: The most recent Tb screening for staff #13 was dated 3/31/17 and was, therefore, over two years old.

Plan of Correction: Staff will obtain Tb screening immediately. All staff will obtain updated Tb screenings when required.

Standard #: 22VAC40-185-270-A
Description: Based on observation, it was determined that the facility did not ensure that all areas of the facility were safe. Evidence: 1. The were dangling cords that posed strangulation hazards in several classrooms of the preschool building. 2. There were at least five holes between one and two feet in diameter on the playground that posed tripping hazards.

Plan of Correction: 1. Staff will be retrained on securing cords. 2. Facility management will work to level out areas of playground to alleviate tripping hazards (9/15/19).

Standard #: 22VAC40-185-280-B
Description: Based on observation, it was determined that the facility did not ensure that all hazardous substances were stored in a locked place. Evidence: 1. There was a bottle of hand-sanitizer (labeled Keep out of reach of children) in one of the school age lockers in the Village. 2. There was a spray bottle of Multipurpose Germicidal Detergent (labeled hazardous and keep out of reach of children) stored in the far left cabinet above the counter in Gallery 89.

Plan of Correction: Staff will be retrained to keep all hazardous substances in locked locations.

Standard #: 22VAC40-185-510-G
Description: Based on a review of medication, it was determined that the facility did not ensure that all medication had parental authorization. Evidence: Although the epi-pen for child #1 had physician authorization, it did not have the required parental authorization.

Plan of Correction: Facility nurse will obtain parental authorization and place it in the medication binder.

Standard #: 22VAC40-185-570-E
Description: Based on observation, it was determined that the facility did not ensure that all prepared infant formula is dated. Evidence: The bottles in the refrigerator of the Get Well Place used by the infants in care in the satellite nursery did not have dates on them .

Plan of Correction: Bottles were dated during the inspection and staff reminded to date all bottles.
