Comments:
An unannounced monitoring inspection was conducted on June 12, 2019 from 9:35am until 10:50am. Upon arrival to the facility, there were 3 children present with 2 staff members. The children were observed participating in age appropriate activities, , hand washing and eating snack. A sample of 4 children's records and 4 staff records were reviewed. The licensing inspector toured the facility and reviewed supervision, emergency supplies, practice drills, nutrition, physical plant, required postings, and administration. There was a violation cited in the area of administration . An exit meeting was conducted with the facility director prior to the end of the inspection.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based upon review of records the provider did not have documentation of a current tuberculosis test on file for staff. Evidence: 1. During review of staff records it was found that staff #3 did not have a current tuberculosis test on file. a. Staff# 3 has a documented date of hire as 5/9/19. b. Staff #2 confirmed that the tuberculosis screening was not on file.

Plan of Correction: Will get staff to submit documentation of tuberculosis test.
