An unannounced monitoring inspection was completed on May 5, 2019 from approximately 9:15a.m. to 11:30a.m. Seventy-six children were in care and eighteen staff were on premises during the inspection. Staff and children were observed in classrooms and on the playground engaged in activities to include activities for infants, outside play, snack time, group reading, art and circle time. Interviews and interactions were conducted and medications were reviewed. Seven children?s records and five staff records were reviewed. The Director was available for the inspection and present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Standard #: 22VAC40-185-550-C Description: Based on observation, the licensee failed to post emergency evacuation and shelter-in-place procedures/maps in a location conspicuous to staff and children on each floor of each building. Evidence: 1. There were no emergency evacuation and shelter in place procedures/maps posted in the building. 2. Administrator #1 advised the building was painted and the procedures/maps did not get re-posted. Plan of Correction: The Director will post the required emergency evacuation and shelter in place procedures/maps.
Standard #: 22VAC40-185-550-M Description: Based on a review of records, the licensee failed to maintain all required elements of a written record of children's serious and minor injuries in which entries are made the day of occurrence. Evidence: 1. Forty-two injury reports were reviewed and approximately half of these lacked all of the required elements. 2. Administrator #1 acknowledged these reports were missing all the required elements. Plan of Correction: The Director will retrain each staff member on the required elements of the injury reports.
