Comments:
An unannounced Complaint Inspection was initiated in the center and completed on June 10, 2019 and completed on June14, 2019 in regards to a complaint received by the department regarding special care provisions/emergencies and supervision. An inspection was conducted at the facility, interviews with staff, review of center documentation and observations were made. There was a preponderance of evidence to support the allegation for supervision therefore it was determined VALID. The area of noncompliance was noted on the violation notice. Standard #: 22VAC40-185-350-E-4
Complaint related: Yes
Description: Based on documentation review and interviews, the licensee did not ensure there was one staff member for every 10 children for children from three years to the age of eligibility to attend public school, five years by September 30. Evidence: In room 107, on June 7, 2019, between 8:15 am and 9:45 am there were eleven up to sixteen children with only one staff member and the ratio required was one staff for every ten children. Child # 1was the youngest child. Center director and Staff #1 confirmed the child to staff ratio on June 7, 2019 was inadequate. Center director also verified the center documentation noted there was only one staff with more than 10 children.

Plan of Correction: That morning there were three staff that called out sick and additional staff was immediately called in to work. Both center director and assistant were working in a classroom until additional staff arrived. Moving forward, the cook who is qualified will be trained to step in as needed.

Description: Based on documentation review and staff interviews, the licensee did not ensure that the center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence and all required documentation was included. Evidence: Four out of seven injury reports were reviewed and determined incomplete as they did not include the following; one did not include how the parent was notified of the injury, one did not not include the time notified of the injury, one did not include the treatment for the injury, one did not include the prevention plan and one did not include who was notified of the injury.

Plan of Correction: The center director will retrain the staff. The assistant director talked to each staff on 6-11-2019 and a formal training will be on 7-9-19.
