An unannounced monitoring inspection was conducted today, June 13, 2019 to determine the center's compliance with licensing standards. The inspection was initiated at 8:50 am and concluded at 11:30 am. Children were observed in a variety of activities. Children in the two and three year old classrooms were combined for music class where they sang songs. The five preschool classrooms were combined for music and movement in one of the larger rooms where they did different dance steps along with a video. Later children returned to their individual classrooms where they listened to stories, completed projects for Father's Day gifts, drew self portraits, and played in centers. All classrooms were inspected. The outdoor playground area and pool were not inspected due to inclement weather . An inspection of the pool will be conducted over the summer months. The pool gate was locked. The center had the following information posted; emergency numbers, evacuation maps/procedures and daily schedules. Children provide their own meals. The first aid kits for each building and floor were checked and found complete along with the non-medical emergency supplies. The inspector interacted with staff and children in care. Seven children?s records and three staff records were reviewed during today's inspection. A follow-up of one staff records was also conducted. Per the director: medication is not being administered. Last emergency drill: 05/02/2019 Last shelter-in-place drills: 01/09/2019, 03/06/2019 Today census: Twos- 7 children with 2 staff. Preschool (young threes) 8 children with 2 staff. Pre-K- ( 5 year old) 9 children with 1 staff. Pre-K- (rising kindergarten) 11 children with 1 staff. Pre-K- (four year old) 11 children with 2 staff. Standard #: 22VAC40-185-160-C Description: Based on review of staff records, the center did not obtain documentation of a follow-up tuberculosis screening at least every two years from the date of the first initial screening or testing. Evidence: The most recent tuberculosis screening for staff #4 was dated 02/27/2017, exceeding the two year allowance to renew. Plan of Correction: Per the director: The staff will have it e-mailed.
Standard #: 22VAC40-185-40-D Description: Based on observation and interview, the center did not post the license in a place conspicuous to the public. Evidence: 1. The license was not posted in the center. 2. The director located an expired license and stated she could not locate the current license. Plan of Correction: The director requested the Department provide her with a duplicate copy of the license. Per the director: We will post the license at the entrance.
Standard #: 22VAC40-185-70-A Description: Based on review of staff records, the center did not ensure each staff record contained all required documentation. Evidence: 1. The record of staff #1, staff #2 and staff #3, all hired 06/10/2019 and identified as a program leaders, did not contain written information to demonstrate the individuals possess the education and experience required by the job position. 2. The record of staff #2 and staff #3, both hired on 06/10/2019, did not contain documentation of two or more references as to character and reputation as well as competency that were checked before employment. Plan of Correction: Per the director: We will add the transcript to the file.
