Comments:
The licensing inspector conducted an unannounced monitoring inspection Thursday, June 13, 2019 from 11:17am to 2:10pm. The children were observed finishing lunch and preparing for rest time. The infants were observed sleeping and finishing their lunch. The playground and the facility?s mini bus and van were inspected today. Five children?s records and five employee records were reviewed during this inspection. Medication is administered and was inspected. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 01/22/2019 Last shelter-in-place drill: 09/13/2019 Last fire inspection: 01/29/2019 Last health inspection: 02/01/2019 Total enrollment today: 54 Today, the following child to staff ratios were observed: Infants-3:1 Toddlers ?3:1 Two's-Preschool ?32:4 Pre-K-10:1 School Age ? 6:1 The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-500-B
Description: Based on observation on 06/13/2019, the center did not ensure that a nonabsorbent surface for diapering or changing shall be used. EVIDENCE: The diapering pad used in the infant and toddler rooms was torn and the nonabsorbent foam cushioning was exposed.

Plan of Correction: Corrected on site. Mat was replaced with a new one.

Standard #: 22VAC40-185-510-N
Description: Based on a review of medication on 06/13/2019, the center did not ensure that when an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center. EVIDENCE: Albuterol belonging to Child #6 was observed at the center. The authorization for the medication expired on 02/18/2019.

Plan of Correction: The parent has been sent an email today. I will have her complete the authorization form at pick up today.

Standard #: 22VAC40-185-540-C
Description: Based on observation on 06/13/2019, the center did not ensure that the required first aid kits contained the minimum items. EVIDENCE: The first aid kit on the vehicle used to transport the children did not contain more than one triangle bandage.

Plan of Correction: Corrected on site. Additional bandage placed in bus
