Comments:
An unannounced monitoring inspection was conducted with the center owner and staff on June 13, 2019 to determine compliance with the licensing standards. There were 30 children present, ranging in age from 5 months to 5 years, with 6 staff actively supervising. The children were having rest time and waking for afternoon snack. Snack served today: graham crackers and apple juice. There are reportedly no medications on site. The last health inspection was conducted 3/9/2019. The last fire inspection was conducted 4/18/2019. Five children's files and four new staff files were reviewed and are in compliance - please see Supplmental Summary for details. Violations:
Standard #: 22VAC40-185-450-A
Description: Based on observation, the facility failed to have linens consisting of a top cover and a bottom cover for cots used by children during rest time. Evidence: a preschool child (age 3) did not have a top or bottom cover during rest time today.

Plan of Correction: Center director has asked parents repeatedly to supply top and bottom covers for rest time.

Standard #: 22VAC40-185-550-D
Description: Based on review of the emergency drill log and staff interview, the facility failed to have a practice evacuation drill monthly. Evidence: there was no drill documented for March 2019 and a drill was not conducted in May 2019.

Plan of Correction: Evacuation drills will be resumed June 2019 and will be documented.

Standard #: 22VAC40-185-570-B
Description: Based on observation, the facility failed to prevent bottles to be used while the child is in his designated sleeping location. Evidence: one toddler was observed on his rest cot with a bottle.

Plan of Correction: Children will not be allowed to use bottles while in the designated sleeping location.
