Comments:
An unannounced Monitoring Inspection was conducted today from 10:30am to 1:00pm. There were 82 children (Infants to School-Age) directly supervised by 12 staff. The physical plant, 4 staff records, 9 children?s records, 4 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in circle time, painting, playing freely, arriving to care, interacting with staff, and preparing for lunch. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Description: Based on review, the facility failed to ensure that they obtained documentation of additional immunizations once every six months for children under the age of two years. Evidence: Reviewed 3 children under the age of 2 years records (Child 1, 2 and 3) and found that all 3 did not have documentation of additional immunizations within the last 6 months.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the facility failed to ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Evidence: Observed that the window blind cords in the school-age classroom were all knotted, creating a large loop, hanging low along the window and accessible to the children in the school-age classroom.

Description: Based on observation, the facility failed to ensure that hazardous substances are kept in a locked place using a safe locking method that prevents access by children. Evidence: 1) Observed in the School-Age Room, a cabinet with broken safety locks and 3 disinfectant sprays, accessible to children. 2) Observed in the Pre-K Room, a cabinet with broken safety locks and 2 disinfectant sprays, accessible to children. 3) Observed in the 2s Room, a cabinet with broken safety locks and at least 4 bottles of disinfectant/sanitizing sprays, accessible to children.

Description: Based on observation, the facility failed to ensure that sinks have running water that does not exceed 120 degrees Farenheit. Evidence: Ran the water in the 3s classroom, and it ran hot, tested the temperature and it ran up to 124.5 degrees F.

Description: Based on observation, the facility failed to ensure that materials and equipment available shall be age and stage appropriate for the children. Evidence: Observed that there were small chokable insects in the 2s classroom.

Standard #: 22VAC40-185-540-B
Description: Based on observation, the facility failed to ensure that first aid kits are easily accessible to staff but not to children. Evidence: Observed a small first aid kits that included bandaids, gloves, antiseptic spray, tweezers and other items that were on a hook, by the shelf, accessible children in the school-age classroom.
