Comments:
An unannounced monitoring inspection was conducted today from 11:00am-1:15pm. There were 39 children (2-5yrs.) supervised by 8 direct care staff. The physical plant, 3 staff records, 4 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed playing sound Bingo, building with blocks, and playing outside on the playground. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-270-A
Description: Based on playground observation, all areas and equipment were not maintained in a clean, safe, and operable condition. Evidence: 1. The large climbing structure on the playground had several areas of peeling finish and rust. 2. The wooden platform for the hand-operated water pump had a wodden board that was deteriorating.

Plan of Correction: Both areas will be covered today. We will have the large climbing structure looked at for appropriate next actions. We will replace the deteriorating wooden board within one month.

Standard #: 22VAC40-185-280-B
Description: Based on classroom observation, 1 of 2 classrooms did not ensure that hazardous substances were kept in a locked place using a safe locking method that prevents access by children. Evidence: The 2 and 3-year old classroom had an unlocked cabinet under the diaper changing station that contained chemical cleaning solutions.

Plan of Correction: Corrected during inspection.

Standard #: 22VAC40-185-510-C
Description: Based on a review of medications, 1 of 6 children's medications did not adhere to methods to prevent use of outdated medication. Evidence: Child #1's (start date: 9/5/2017) Auvi-Q expired 1/23/2019.

Plan of Correction: We will contact the parents to bring in a replacement.

Standard #: 22VAC40-185-550-M
Description: Based on a review of injury reports, 1 of 5 injury reports did not contain documentation of staff and parent signatures or two staff signatures. Evidence: 1 injury report contained documentation of only one staff signature.

Plan of Correction: Corrected during inspection.
