Comments:
An unannounced mandated inspection took place on 06/13/2019 between the hours of 8:15am and 12:00pm. On this date, there were 53 children in care between the ages of 3 months and 4 years old. The inspector observed the children eating breakfast, engage in free play, play on the playground, and eat lunch. The inspector reviewed medications, children's records, first aid kits, and drill documentation.

Violations:
Standard #: 22VAC40-185-280-B
Description: Based on observation and interview with staff, the provider failed to store hazardous substances such as cleaning materials, in a locked place using a safe locking method that prevents access by children. Evidence: 1. In Classroom B, cleaning chemicals (identified by staff as bleach) were being stored in cabinets below the sink. The cabinets were unlocked and at eye level to a small child. 2. In Classroom B, a mop with an automatic cleaning chemical dispensing unit was located next to the classroom door, accessible to children. 3. In Classroom C, cleaning chemicals were being stored in cabinets below the sink (window cleaner, dish soap, and bleach), The cabinets were unlocked and at eye level to a small child.

Plan of Correction: Upon speaking with staff. Staff removed the chemicals and locked them immediately. Staff plans to implement a new locking mechanism in Classroom C and administration will oversee proper chemical storage in the future.

Standard #: 22VAC40-185-280-G
Description: Based on observation and interview with staff, the provider failed to clearly indicate contents of substitute containers which contain hazardous substances. Evidence: 1. Classroom B contained two spray bottles with no identification of the contents. Staff stated that one spray bottle was soap and water and one bottle contained a bleach and water solution. The bottles were located in an unlocked cabinet, accessible to children.

Plan of Correction: Staff plans label substitute containers.

Standard #: 22VAC40-185-340-F
Description: Based on observation and interview, the provider failed to keep a child under the age of 10 years in actual sight and sound supervision. Evidence: 1. In Classroom D, there was a 12-month-old child, left out of sight supervision. This child could not be seen by staff and was out of sight for at approximately 15 minutes.

Plan of Correction: Staff plans to have to children located closer to teachers.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the provider failed to provide a nonabsorbent surface for diapering or changing. Evidence: 1. The diaper changing pad located in Classroom E, had a tear in it, which makes the pad absorbent.

Plan of Correction: The staff plans to replace the diaper changing pad.
