Comments:
An unannounced monitoring inspection was conducted on Tuesday, June 11, 2019 to determine the center's compliance with licensing standards. The inspector arrived at the center at 10:35am and concluded the inspection at 1:52pm. The center's director assisted the licensing inspector throughout the inspection. The census for today?s inspection consisted of 128 children in the direct care of 23 staff. There were additional staff available on site during the inspection. Upon the inspector?s arrival, the children were engaged in a variety of activities. The theme of the week was transportation and sea animals. The children in the Yellow Room were formally observed in their designated primary care groups. A group of children were observed interacting with peers and staff while they played with blocks and legos. Another group was observed taking bathroom breaks, washing hands, and later reading a book called "Secrets of Animal Camouflage." The children in the Blue Room were formally observed during transitions from free-play, bathroom breaks, and later reading a story about ocean animals. The children in the Purple Room were observed during a teacher-led activity and discussing facts about the ocean. The children in the Gold Room were observed entering their classroom from the playground, washing hands, and later interacting with staff during a teacher-led activity. Staff were observed having positive interactions with the children. The playground was inspected. The classrooms are decorated with the children?s artwork and are equipped with age and stage appropriate materials. The center had the following posted: license, daily schedule, emergency numbers, evacuation maps, menu and various parent information. The first aid-kit and non-emergency supplies were inspected and found complete. Ten children's records, four medications and authorizations, and ten staff's records were reviewed. The most recently documented fire inspection was on 11/20/2018. The most recently documented health inspection was on 03/08/2019. The most recent practice evacuation drill was documented on 06/05/2019. The most recent practice shelter-in-place drill was documented on 06/07/2019. The menu was posted and today's lunch consisted of BBQ chicken sliders on a WG roll, fresh carrots, cantaloupe, and milk.

Please complete the 'plan of correction' and 'date to be corrected' for each violation cited on the violation notice and return it to me within 5 business days from the date of receipt. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standards, 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Description: Based on record review and interview on 06/11/2019, the center did not implement a monthly practice evacuation drill.

Evidence: 1) The licensing inspector observed that the most recent evacuation drill was documented on 06/05/2019. 2) The previous evacuation drill was documented on 04/26/2019. There was no drill documented during the month of May 2019. 3) A member of management confirmed an evacuation drill was not conducted in May 2019. The center shall implement a monthly practice evacuation drill and a minimum of two shelter-in-place practice drills per year for the most likely to occur scenarios.

Plan of Correction: Per the Center: Emergency planning is very important to our school. Our school implements monthly fire drills, which is required by the department of social services, and quarterly secondary drills, which exceeds the requirements of the department of social services. Due to unforeseen circumstances, the drill for May 2019 did not occur as intended. The administration immediately identified this and rectified it by conducting a fire drill on June 5, 2019. A second fire drill will be implemented in June 2019. The June 5, 2019 drill was a makeup drill for May and the second June drill will count as our official June drill. While we understand the application of the violation, LeafSpring School at Wyndham will be in compliance for holding 12 drills annually, understanding that the May drill was executed in June. Site administration will continue to monitor this as part of the monthly Quality Assurance meeting where all drills are reviewed and action plans post drill review are taken as necessary.
