Comments:
The licensing inspector conducted an unannounced monitoring inspection Friday, June 14, 2019 from 08:23am to 12:30pm. The children were observed arriving to school, eating breakfast snack and preparing for the graduation this afternoon. The infants were observed playing on the floor and sleeping. All classrooms were inspected today, as well as the playgrounds. Due to school age bus runs the facilities mini buses were not inspected. Six children?s records and six employee records were reviewed during this inspection. The center administers medication and it was reviewed. Last emergency drill: 05/14/2019 Last shelter-in-place drills: 04/05/2019 Last fire inspection: 05/29/2018 Last health inspection: 06/03/2019 Today, the following child to staff ratios were observed: Infants ?7:2 Toddlers ?10:2 Twos ? 10:2 Preschool (Threes) ?14:2 Pre-K ?15:2 School Age ?14:1 The violations from the previous inspections were checked for corrections. There was one repeat violation found during today?s inspection. If you have any questions about this inspection, please contact your licensing inspector, Roshon Casey, at (804) 662-9760. PROVIDER: Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Description: Based on a review of staff records on 06/14/2019, the center did not ensure that each record contained documentation that two or more references as to character and reputation as well as competency were checked before employment. EVIDENCE: 1. The reference checks in the record of Staff #4, whose date of hire was 05/08/2019, are dated 05/13/2019. 2. The reference checks in the record of Staff #5, whose date of hire was 05/13/2019 are dated 05/17/2019.

Description: Based on staff interview, the center did not ensure that an annual fire inspection report was obtained from the appropriate fire official. EVIDENCE: Staff #7 contacted the local fire department during the inspection. Per that conversation with the fire official, Staff #7 stated that the last fire inspection was completed on 05/29/2018.

Standard #: 22VAC40-185-270-A
Description: Based on observation on 06/14/2019, the center did not ensure that areas and equipment of the center shall be maintained in a safe condition. EVIDENCE: 1. A hook on the back wall of the school age classroom is broken. 2. The couch in the preschool room is torn, exposing the foam cushioning.
