Comments:
The licensing inspector conducted an unannounced subsidy health and safety inspection Friday, June 14, 2019 from 08:23am to 12:30pm to review supplemental health and safety requirements. The children were observed arriving to school, eating breakfast snack and preparing for the graduation this afternoon. The infants were observed playing on the floor and sleeping. All classrooms were inspected today, as well as the playgrounds. Due to school age bus runs the facilities mini buses were not inspected. Six children?s records and six employee records were reviewed during this inspection. The center administers medication and it was reviewed. Last emergency drill: 05/14/2019 Last shelter-in-place drills: 04/05/2019 Last lockdown drill: 03/21/2019 Last fire inspection: 05/29/2018 Last health inspection: 06/03/2019 Today, the following child to staff ratios were observed: Infants ?7:2 Toddlers ?10:2 Twos ? 10:2 Preschool (Threes) ?14:2 Pre-K ?15:2 School Age ?14:1 A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. If you have any questions about this inspection, please contact your licensing inspector, Roshon Casey, at (804) 662-9760. PROVIDER: Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Description: Based on a review of staff records on 06/14/2019, the center did not ensure that staff shall be evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB) which shall be submitted at the time of employment and prior to coming into contact with children. EVIDENCE: The tb screening in the record of Staff # 3, whose date of hire was 12/11/2018, is dated 03/04/2019.
