Comments:
An announced monitoring inspection was conducted with the curriculum specialist and staff. There were 123 Children in care and the age range of the children was seven months to ten years old. Children were observed during lunch time, outside time and free play. Some of the school age children were on a field trip. Five staff and children's records were reviewed. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation not all areas were maintained in safe and operable condition. Evidence: On the Nursery playground, the Pre-school playground, and the school age playground had weed block sticking up posing a tripping hazard. On the school age playground there was a yellow extension cord stretched across the out door Theater posing a tripping hazard to the children.

Plan of Correction: We will contact the maintenance person.

Standard #: 22VAC40-185-370-1
Description: Based observation and staff interviews, staff is not placing infants in cribs on their backs. Evidence: Staff in the infant room stated they placed two infants in cribs on their stomachs for rest time.

Plan of Correction: We will retrain all infants staff on Monday morning and will speak with all staff that are today.

Standard #: 22VAC40-665-620-A
Description: Based on observation not all hazardous substances were kept in a lock area. Evidence: In the art room there were two bottles of equate lotion and vegetable glycerin in an unlocked file cabinet and in the Explores room there was bleach water in stored in an unlocked cabinet. all with keep out of the reach of children warning labels.

Plan of Correction: All were removed during the inspection and placed in locked areas.
