Comments:
An unannounced monitoring inspection was conducted on Friday, May 31, 2019 to determine the center's compliance with licensing standards. The inspector arrived at 3:00 PM and concluded the inspection at 5:30 PM. The center?s director and assistant director assisted the inspector throughout this inspection. The census for today?s inspection consisted of 23 children. During the inspection, children and staff were observed participating in a variety of activities such as puzzles, story time, singing and building with large Legos. The classrooms are decorated with the children's artwork and are equipped with age and stage appropriate materials. All areas of the center including the classrooms, hallways, bathrooms, and playgrounds were inspected. The center had all required information posted. The center?s first aid kit and nonmedical emergency supplies were inspected. Five children's records and five staff records were reviewed during today's inspection. Violations were cited and are documented on the violation notice. Health Inspection: 06/28/2018 Fire Inspection: 03/04/2019. Standard #: 22VAC40-185-60-A
Description: Based on review of children's records, the center did not ensure that each child's record contains all required information. Evidence: The record for child #3 did not contain the phone number of child's physician.

Plan of Correction: "Child's file will be updated corrected today."

Standard #: 22VAC40-185-70-A
Description: Based on a review of staff records, the center did not ensure that one of five records reviewed contained documentation that two or more references as to character and reputation as well as competency were checked before employment. Evidence: The record for staff #1 (date of hire was 03/05/2019) did not contain documentation of two or more references.

Plan of Correction: "Staff member file will be updated as soon as possible."

Standard #: 22VAC40-185-240-A
Description: Based on review of staff records, the center did not ensure all staff received required training by the end of their first day of assuming job responsibilities. Evidence: Staff #1 (date of hire 03/19/19) does not have documentation of required training.

Plan of Correction: "Staff member file will be updated as soon as possible."

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not ensure that all hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. Evidence: The kitchen cabinet under the sink had Clorox wipes labeled "Harmful" and "Hazards to humans," Windex labeled "Danger" and "Keep out of reach of children," and Lysol and two cans of disinfectant spray labeled "Keep out of reach of children." The cabinet was not locked and accessible to children.

Plan of Correction: Corrected during inspection.

Standard #: 22VAC40-185-560-K
Description: Based on observation, the center did not ensure that the surface of tables were sanitized using a spray disinfectant solution or a dipped disinfectant solution before and after each use for feeding. Evidence: The after school teacher used Clorox wipes on the surface of the table. The center is required to sanitize the surface using a spray disinfectant solution or a dipped in the disinfectant solution.

Plan of Correction: "Will plan to go over with staff about proper cleaning supplies at next staff meeting."

Standard #: 22VAC40-191-60-C-2
Description: Based on a review of staff records and an interview with the assistant director, the center did not have documentation of central registry findings for one staff member. Evidence: The record for staff #1 (date of hire 03/05/19) did not contain documentation of central registry findings.

Plan of Correction: Central Registry results was sent to the licensing inspector on Monday, June 3, 2019.
