Comments:
An unannounced monitoring inspection was conducted on Friday, June 7, 2019 to determine the center's compliance with licensing standards. The inspector arrived at 10:45 AM and concluded the inspection at 2:40 PM. The center's director and assistant director assisted the inspector throughout this inspection. The census for today's inspection consisted of 57children. During the inspection, children and staff were observed participating in a variety of activities including having relay races in the gym, singing songs, reading, preparing for lunch, and eating lunch. The classrooms are decorated with the children's artwork and are equipped with age and stage appropriate materials. The classrooms, hallways, cafeteria, and bathrooms were inspected. The center had the following posted: license, daily schedule, emergency numbers, evacuation maps and a menu. Five children's records, five staff records, fire drill, shelter in place and lockdown drills, daily attendance, and nonmedical emergency supplies were reviewed. Standard #: 22VAC40-185-140-A
Description: Based on a review of children's records, the center did not ensure that each child shall have a physical examination by or under the direction of a physician before the child's attendance or within one month after attendance. Evidence: The records for child #1 (date of enrollment 04/22/2010) and child #2 (date of enrollment 10/22/2018), did not have a physical examination.

Plan of Correction: "Request updates w/in next 10 days."

Standard #: 22VAC40-185-160-A
Description: Based on a review of staff records, the center did not ensure that each staff member shall submit documentation of a negative tuberculosis screening (TB) no later than 21 days after employment or volunteering and shall have been completed within 12 months prior to or 21 days after employing or volunteering. Evidence: 1. Staff # 3 (date of hire 10/30/2017 did not have documentation of the TB screening / test. 2. The director and assistance director confirmed that the screening was not in the record for staff #3.

Plan of Correction: "Send over the proof of current TB test w/ next 5 days."

Description: Based on observation, the center did not ensure that all hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. In classroom #1, a spray bottle with bleach and water and two cans of Lysol disinfectant spray labeled "Danger," "Hazards to humans" and "Keep out of reach of children" was on the shelf of an unlocked cabinet. Children in this classroom are under 2 years of age. 2. In classroom #2, one can of Lysol disinfectant spray labeled "Danger," "Hazards to humans" and "Keep out of reach of children" was in an unlocked cabinet. Children in the classroom are 3 years to 4 years of age.

Standard #: 22VAC40-185-290-3
Description: Based on observation, the center did not ensure that child-resistant protective covers shall be installed on all unused electrical outlets accessible to children of preschool age or younger. Evidence: 1. In classroom #1, the electrical outlets were not covered. Infants and young toddlers are in classroom #1. 2. In classroom #1, the electrical outlets were not covered. Preschool age children are in classroom #2. 3. The electrical outlets were not covered in the gym. Preschool age children were observed playing in the gym.

Standard #: 22VAC40-185-550-M
Description: Based on a review of the facility's injury reports, all required information was not documented. Evidence: 1. Eleven injury reports did not have the time of injury and four of the eleven did not have the date of the injury. 2. Three injury reports did not have any future action to prevent recurrence of the injury. 3. Six injury reports did not have documentation of how the parent was notified. 4. The director stated that she was aware that staff were not completing all the required information.

Plan of Correction: "Retrain staff on proper completion of these reports."

Standard #: 22VAC40-80-120-E-2
Description: Based on observation and interview, the center did not ensure the findings of the facility's most recent inspection was posted. Evidence: 1. A posted violation notice was dated 05/22/18. 2. The Director stated she knew she should have put the last inspection notice up. 3. The most recent inspection occurred on May 22, 2019.
