Comments:
A unannounced monitoring inspection was conducted with the director and staff. 113 Children were in care with an age range of eight weeks to five years old. Children were observed during rest time, free play. The staff conducted a fire drill and the staff got all the children out of the building quickly and in a orderly manner. Six staff records and children's records were reviewed. All required paper work was posted. Standard #: 22VAC40-185-450-A
Description: Based on observation not all children were resting on cots that consisted of a top and bottom cover. Evidence: In the Art Studio The was one child resting on a cot with out a bottom cover.

Plan of Correction: Staff will make sure a top cover is provide to every student

Standard #: 22VAC40-185-500-B
Description: Based on observation and interviews with staff , not all rooms with children of diapering age had a system that was foot operated for soiled diapers. Evidence: In the Science Lab There was no step on diaper pail. Staff were throwing the soiled diapers in a regular open trash can.

Plan of Correction: A foot operated trash can was placed in the rest room.

Standard #: 22VAC40-185-510-A
Description: Based on review of children's medication not all had parent Authorization . Evidence: Child A's medication did not have a parents Authorization to give the medication.

Plan of Correction: Parents were given a form to sign for medication.

Standard #: 22VAC40-665-610-A
Description: Based on observation not all doors on cubbies closed properly. Evidence: In Art Studio room there were six cubbies that were not closing properly and are posing a pinching hazard to the children.

Plan of Correction: All cubby doors will be inspected and repaired.
