Comments:
The licensing inspector conducted an unannounced monitoring inspection on Friday, June 14, 2019 from approximately 10:00 AM until 3:30 PM. The following ratios were observed with a total of 94 children present in the Preschool building and 58 children in the Village (school age) building: Satellite Infants - 3 children with 1 staff Younger Infants - 8 children with 2 staff Older Infants - 7 children with 2 staff Toddlers (Red Room) ? 15 children with 2 staff Twos (Yellow Room) - 12 children with 2 staff Twos & Threes (Green & Orange Room were combined) - 17 children with 2 staff Threes & Fours (Blue Room) - 19 children with 2 staff Fours and Five (Purple Room & Gold Room were combined) - 16 children with 2 staff Get Well Place ? no children were present School age children - 58 children with 5 staff The children and staff were engaged in a variety of activities while the licensing inspector was on the premises. Children were observed playing outside, being held and rocked, playing with toys, painting pictures of the flag, drawing pictures of their favorite means of transportation, working on computers, building with blocks, having their diapers changed, taking bathroom breaks and washing their hands and eating lunch. Lunch consisted of turkey pitas with lettuce, peaches, cheesy cauliflower, mixed vegetables and milk. Later, children were observed resting at nap time. All areas of the center including classrooms, hallways, kitchen, bathrooms and outdoor play areas were inspected. The center was equipped with toys and supplies and items were available to the children. The following information was reviewed: infant feeding schedules, daily attendance, infant daily sheets, practiced emergency drill logs, liability insurance policy, parent agreements, staff training information, required postings and menu. The center?s first aid kit and emergency supplies were inspected. Medication and medication authorization forms were reviewed. Transportation documents and vehicles were inspected. During the inspection, 10 children's and 10 staff records were reviewed. Standard #: 22VAC40-185-70-A
Description: Based on review of 10 staff records, the center did not ensure three staff records contained the required information. Evidence: 1. The records for Staff #7 and Staff #8 did not contain documentation of education required for job position. Both staff members were identified as a program leader during the inspection. 2. The record for Staff #9 contained documentation of one reference check.

Plan of Correction: The information will be added to the staff records.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the center did not ensure areas and equipment, inside and outside, were maintained in a clean, safe and operable condition. Evidence: The playground door leading to the Blue Room contained peeling paint and areas of rust.

Plan of Correction: The door will be sanded and painted.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not ensure all hazardous substances were kept in a locked place using a safe locking method that prevented access by children. Evidence: 1. A cabinet located above the sink in the Red Room contained cleaning chemicals. The cabinet was unlocked. 2. 1. A cabinet located above the sink in the Yellow Room contained cleaning chemicals. The cabinet was unlocked.

Standard #: 63.2(17)-1721.1-B-3
Description: Based on review of 10 staff records, the center did not ensure a child abuse and neglect search maintained by another state was requested for two staff members what have lived outside of Virginia within the last five years was requested. Evidence: The records for Staff #7 (hire date 05/20/19) and Staff #8 (hire date 04/30/19) indicated the staff had lived outside of Virginia in the last five years. Members of management confirmed during the inspection a request for a child abuse and neglect search had not been made to the states the staff members documented they had lived.

Plan of Correction: The information was requested while the licensing inspector was on site.
