Comments:
A Subsidy Health and Safety inspection was conducted today at 9:45am to 1:00pm to review supplemental health and safety requirements. There were 84 children under the supervision of 13 staff. Children were observed playing outside, playing freely inside, some infants were napping, and children were preparing for lunch.. There was a sufficient supply of books, toys, and materials that were developmentally appropriate for the children in care. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: 22VAC40-665-580-B
Description: Based on review, the facility failed to ensure that staff obtained documentation of completing within 90 days of employment, Virginia Preservice Training. Evidence: Reviewed 5 staff records and found that Staff 1, 4 and 5 have been on site for more than 90 days and did not have documentation of completed Virginia Preservice Training.

Plan of Correction: An unexpected staffing emergency required the director to have to drive a bus during a field trip. The new supervisor was not familiar with the filing system and was unable to locate some of the documents during the visit. All documentation of the Virginia Preservice training was on site and emailed to the licensing inspector when the director returned from the field trip.

Description: Based on Review, all staff who work directly with children shall have 90 days to obtain documentation of current certification in CPR. Evidence: Reviewed 5 staff records and found that Staff 1 and 5 did not have documentation of current CPR.

Plan of Correction: Staff 1 and 5 have completed the on-line portion and are scheduled for the hands on portion Friday, June 14, 2019.

Description: Based on review, all staff who work directly with children must obtain documentation of certification of First Aid training within 90 days of employment. Evidence: Reviewed 5 staff records and found that Staff 1 and Staff 5 did not have documentation of first aid training.

Plan of Correction: Staff 1 and 5 have completed the on-line portion and are scheduled for the hands on portion Friday, June 14, 2019.
