Comments:
Thank you for your assistance during the renewal inspection conducted from 12:11 PM to 6:55 PM. Today, there were fifty-five children in care, ages two through five years, with eleven staff. Twelve children, ages one through three years, were in the care of four staff during extended hours of care. Four school age children received evening care at the center with two staff. I viewed program activities, daily routines, staffing, supervision, interactions with children, equipment, agency inspections, posted information, emergency planning, transportation, injury records, emergency supplies and drill records, nutrition, menu, snacks/dinner, medication, seven records for children, seven staff records, staff qualifications, staff training, indoor and outdoor areas. The teachers have offered a variety of art experiences and group activities according to themes such as "Jungle Week". There was a large selection of age appropriate toys, equipment and materials in interest areas of each room. Regular outside activities have been offered, including during evening hours. Standard #: 22VAC40-185-70-A
Description: Based on a review of seven records for staff, and interviews with administrative staff, documentation of all required staff development for the job position was not in the record of one staff. Evidence: The Virginia Preservice Training certificate was not in the file of staff member 2 who began work on November 30, 2018. Administrative staff indicated they saw staff member 2 working on the ten hour course on a computer at the center. During the inspection, staff member 2 provided information for accessing a copy of the certificate and it could not be accessed. When requested, the certificate was not brought from home on June 13 or June 14, 2019.

Plan of Correction: Administrative staff told staff member 2 to submit a copy of the Virginia Preservice Training that has been completed as claimed. The director will resume using a tracking system for the needed ten hour training certificate so that new staff submit the information for the staff records within ninety days of initial employment.

Standard #: 22VAC40-185-290-3
Description: Based on observation of the activity areas, and an interview with staff, there was an electric outlet without a protective cover in one of the preschool age classrooms. Evidence: During the nap time, an outlet at a rug area did not have a protective cover in Room 3 where seventeen children were in attendance, ages three to five years. Staff indicated they did not know why it was left uncovered.

Plan of Correction: The outlet cap was located on a cart next to the rug area wall with the outlet and the outlet was covered immediately. The staff will make sure outlets are covered after use. Center administration will remind staff about the requirement for covering electric outlets when they are not in use.
