Comments:
A complaint investigation was conducted on June 13, 2019 regarding ratios in the Infant classroom. Interviews were conducted, documentation was reviewed, and observations were made. There was a preponderance of evidence to support the allegation regarding supervision and the complaint is deemed valid. Related violations are identified in the Violation Notice. Description: Based on review of attendance records, the center did not maintain a ratio of one staff member for every four children for children from birth to the age of 16 months. Evidence: On 6/4/2019, name-to-face record for the Infant A classroom (children ages 6 weeks to 16 months) documented 11 children and 2 staff from approximately 8:00am-8:30am.

Plan of Correction: We are having a mandatory staff meeting tonight to go over the Child Supervision Record and the importance of making sure all children and staff are signed in. We will also be going over the staff to child ratio for all classrooms and the importance of knowing how many children are in your class. We will also be instructing staff to let management know when they are reaching the maximum ratio for the number of teachers in the class. As Management, we will continue doing periodic CSR checking throughout the day.

On 5/28/2019, the name-to-face record for the Discovery Preschool classroom documented 17 children and 2 staff from approximately 10:00am-12:00pm. 3. On 5/31/2019, the name-to-face record for the Discovery Preschool classroom documented 17 children and 2 staff from approximately 10:00am-11:15am. 4. On 6/4/2019, the name-to-face record for the Discovery Preschool classroom documented 20 children and 2 staff from approximately 9:00am-9:10am. 5. On 6/5/2019, the name-to-face record for the Discovery Preschool classroom documented 19 children and 2 staff from approximately 9:00am-9:10am. From approximately 10:45am-1:00pm, the name-to-face record documented 17 children and 2 staff. From approximately 3:30pm-3:59pm, the name-to-face record documented 17 children and 2 staff. 6. On 6/6/2019, the name-to-face record for the Discovery Preschool classroom documented 20 children and 2 staff from approximately 9:00am-10:00am. From approximately 10:00am-10:17am, the name-to-face record documented 21 children and 2 staff. From approximately 10:45am-1:00pm, the name-to-face record documented 17 children and 2 staff. 7. On 6/11/2019, the name-to-face record for the Discovery Preschool classroom documented 18 children and 2 staff from approximately 8:30am-8:58am. From approximately 8:58am-10:13am, the name-to-face record documented 17 children and 2 staff. 8. On 6/12/2019, the name-to-face record for the Discovery Preschool classroom documented 19 children and 2 staff from approximately 8:31am-8:37am. From approximately 8:41am-9:01am, the name-to-face record documented 17 children and 2 staff. From approximately 8:59am-9:16pm, the name-to-face record documented 17 children and 2 staff. 9. During the time of inspection, the Discovery Preschool classroom was observed with 17 children and 2 staff from approximately 8:31am-8:35am.
