63.2(17) License & Registration Procedures
An unannounced monitoring inspection was conducted from 11:30 a.m. - 1:30 p.m. There were 132 children being supervised by 20 staff members. There were 128 children off-site at various field trip locations being supervised by 24 staff members. The children were observed participating in outdoor activities, cooperative games, and eating lunch. All meals are provided from home. Fifteen children's records were reviewed and in compliance. Ten staff records were reviewed. Five medications were reviewed and in compliance. Standard #: 22VAC40-185-160-A Description: Based on review of ten staff records it was determined that not all staff records contained documentation of a TB (tuberculosis) screening within the first 21 days of employment. Evidence: Staff C (start date: 06/01/2018) and Staff D (start date: 06/16/2016) did not have documentation of a TB screening on file. Plan of Correction: Administration will request a TB screening from Staff C and Staff D.
Standard #: 22VAC40-185-160-C Description: Based on review of ten staff records it was determined that not all staff repeated a TB (tuberculosis) screening every two years. Evidence: The most recent TB screening on file for Staff A was dated 06/02/2017. The most recent TB screening on file for Staff B was dated 06/15/2017. Plan of Correction: Administration will request that Staff A and Staff B obtain a TB screening and submit the documentation for the file.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on review of ten staff records it was determined that not all staff records contained an eligible determination letter based on fingerprinting on file before the first day of employment. Evidence: Staff A (start date: 06/01/2017) did not have documentation of an eligible determination letter on file. Plan of Correction: Administration is attempting to located the determination letter. If the letter cannot be found, Staff A will be required to obtain a new determination letter.
