Comments:
The licensing inspector conducted an unannounced monitoring inspection, at which play groups, activities, centers, art, story time, lunch time, rest time, and personal hygiene were observed. Children were observed; and staff and children files and other documentation were reviewed. Forty-six (46) children, aged 6 months-8 years, were found to be in care with 7 staff and office staff. Previous violations were reviewed, and had been corrected. Please contact me if you need further assistance. Thank you for your cooperation. This inspection began at 8:45am and concluded at 1:15pm.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on review of children records the facility failed to maintain compliance with this standard as required relating to items required for emergency contacts. Evidence: The record for child #2 did not have emergency contact phone numbers as required.

Plan of Correction: These numbers will be obtained by the end of the day today, and documented within the file for compliance. All files will be reviewed for compliance.

Standard #: 22VAC40-185-280-B
Description: Based on review of the facility the facility failed to maintain compliance with this standard as required relating to locking of hazardous substances or areas. Evidence: In the school aged hallway, near the kitchen, there was a storage closet containing numerous cleaning agents that was unlocked and accessible by the school age children and preschool aged children in care. Also, on the opposite side of the wall from, the above mentioned closet, there was a storage cabinet that was unlocked and accessible to the same children that contained cans of interior wall paint.

Plan of Correction: Both areas will be locked and staff reminded of requirements of locking of hazardous items at all times.
