63.2(17) License & Registration Procedures

Comments:
A announced monitoring inspection was completed with the director and staff. 60 children were in care with an age range of four months to ten years old. Children were observed during outside time, art projects and free play. Six staff and children's records were reviewed. Six staff are current in MAT. All staff are current in First Aid and CPR. Standard #: 22VAC40-185-330-B
Description: Based on observation not all equipment has resilient surfacing under the slides. Evidence: The resilient surfacing at the end of the large red slide is separated and is not resilient.

Plan of Correction: The head of maintenance was on site today and said it would be fixed by August. The slide will be off limits until it is fixed.

Description: Based on observation the center did not dispose of soiled diapers in a foot operated storage system. Evidence: Pre-school A room was using a open trash can to dispose of soiled diapers .In Infant B room the staff used their hand to open the step on diaper pail.

Plan of Correction: We will buy a step on diaper pail and we will retrain staff on diaper changing procedures.

Standard #: 63.2(17)-1720.1-A
Description: Based on review of six staff records not all staff had the required search of central registry with in 30 days of hire. Evidence: Staff A with a hire date of 5/7/19 did not have a search of central registry check.

Plan of Correction: We called the search of central registry to ask about the check on 6/18/2019.
