Comments:
A Subsidy Health and Safety inspection was conducted from 10:37 AM to 3:50 PM to review supplemental health and safety requirements. Today, there were thirty-seven children, ages infancy through five years, with seven to eight staff. I viewed record keeping, operational responsibilities, food service, children's information, caregiver training, emergency planning and drills, hazardous substance practices, behavioral guidance, hand washing, five records for children, and five staff records. No medication or food allergies at this time. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. A violation was found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description: Based on a review of five records for staff, and interviews with administrative staff, the center failed to obtain documentation of a tuberculosis screening completed within the last thirty days of the date one staff began working with the children. Evidence: According to staff interviews, staff member 2 began regular work with children in the center on March 11, 2019 and the tuberculosis screening and statement was not completed until March 20, 2019. Staff member 2 was observed by the licensing inspector while working with infants on March 22, 2019.

Plan of Correction: The administrative staff who arrange for staff to take positions working with children at the center will make sure new staff provide a TB statement that is within thirty days prior to contact with children at the center. The administrative staff will review the subsidy requirement for TB statements.
