An unannounced monitoring inspection was completed on 6/19/2019. There were seven children in care with three staff present. The children's ages ranged from 3 years to 12 years. The inspector arrived at 6:30 A.M. and departed at 10 A.M. Six children's records and five staff's records were reviewed during the inspection. Observed the children during: arrival, morning snack, free play, story time, and hand washing procedures. Discussion with the staff about lead teacher qualifications, physical plant plans for outside, children's records, and subsidy requirements.
Standard #: 22VAC40-185-150-B Description: Based on review of six children's records, the center failed to ensure that all immunization records were signed by a physician or a physician designee. Evidence: 1 . Children # 1 and 5 did not have signature from the physician or physician designee on their immunizations forms Plan of Correction: The director will have the parent to obtain signed copies of the immunizations records.
Standard #: 22VAC40-191-60-C-2 Description: Based on review of five staff's records, the center failed to ensure that all new hires had a CPS check within 30 days of hire. Evidence: 1. Staff # 1 was hired on 12/18/2018 and on the day of the inspection the CPS check was not located. There was no documentation of contact with the OBI office to verify the status of the check. Plan of Correction: The director will verify the back ground check status and in the future will follow through with time frames.
