Technical Assistance:
Technical assistance today included record keeping; documentation of training; and physical environment.

Comments:
An unannounced renewal inspection was conducted today from 10:20 am - 1:00 pm. There were 9 preschool children present with the teacher and assistant. I observed outdoor play and drawing with chalk; and later I observed preparation for naptime. Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records today, there was some information not documented as required. Evidence: Of 8 files reviewed, none of the children had information as required about previous child care or concurrent programs; Child # 1, 7 and 8 did not have complete information for two emergency contacts with addresses; and proof of identification did not have documentation of being verified for Child # 1, 2, 3, 6.

Plan of Correction: I will use the DSS application so it will cover everything and I will give it to all parents.
