Comments:
An unannounced complaint investigation was begun in response to a potential violation of standards. Based on multiple interviews, the preponderance of evidence does not support the allegation. The complaint is not valid. Additional violations were cited that were not part of the original complaint, but found during the course of the investigation. Description: Based on a review of records, one staff member's record does not include all required components. Evidence: Staff A's record does not include documentation of: emergency contact, date of employment.

Plan of Correction: I can have the emergency contact form and the date of hire placed in her file.

Standard #: 22VAC40-185-240-A
Complaint related: No
Description: Based on a review of records, one staff record is incomplete. Evidence: Staff A does not have documentation of orientation having been completed by the end of the first day of working with children. (Date of Hire(DOH): October 2018)

Plan of Correction: Orientation was conducted, just not documented. Documentation will be placed in the file.

Complaint related: No
Description: Based on direct observation, a ratio of 1staff /18 children was not maintained in the Pre-K room for children aged 4-5. Evidence: At LI arrival at approximately 2:15 PM, the Pre-K room was out of ratio at 1 staff to 29 children.

Plan of Correction: Ratios will be maintained

Complaint related: No
Description: Based on direct observation, an infant that was not being held during feeding was not placed in a high chair, infant carrier, or feeding table. Evidence: On 6/17/19 an infant was observed lying on the floor, on a Boppy pillow drinking a from a bottle.

Plan of Correction: I will retrain staff on proper feeding procedures. One specific staff member was retrained immediately.

Complaint related: No
Description: Based on direct observation, one vehicle used for transporting children did not meet safety standards set by the Department of Motor Vehicles. Evidence: The license plates for one vehicle are expired. The license plate tags on the vehicle expired in October 2018.

Plan of Correction: The updated tags were placed on the vehicle during the inspection.

Description: Based on a review of records, one staff record does not include documentation of a completed Central Registry Search (CPS), within 30 days of employment. Evidence: Staff A's (DOH10/2018) record does not include documentation of a completed CPS search.

Plan of Correction: I have called the Office of Background Investigations, another copy of the results will be sent to us. If one is not received in 7-10 business days another form will be sent, per OBI instructions.
