Comments:
An unannounced renewal inspection was conducted on Wednesday, June 19, 2019 to determine the center?s compliance with licensing standards. The inspector arrived at the center at 1:45pm and concluded the inspection at approximately 4:10pm. The center's owner, director, and assistant director were present and assisted the inspector throughout the inspection. The census for today?s inspection consisted of 77 children in the direct care of 13 staff. Upon the inspector?s arrival, the children were engaged in a variety of activities. The children in the Infant Room were observed interacting with staff, being fed, and playing with sensory toys on the carpet. A group of children from the Toddler Room were observed in their designated classroom, while the others were observed out on the playground. The children in the Twos Room were observed eating snack. The children in the Threes Room were observed playing, and dancing out on the playground. The children in the Preschool Room were observed during their designated free-choice period. The children were observed interacting with peers and staff, coloring, building with blocks, and logs. The children in the School Age Room were observed during their free-choice period. Staff were observed having positive interactions with the children throughout the inspection. The classrooms are decorated with the children?s artwork and are equipped with age and stage appropriate materials. The playground and two buses were inspected and found to be in compliance. The center had the following posted: license, daily schedule, emergency numbers, evacuation maps, menu and various parent information. The center?s first aid kit and non-emergency supplies were inspected and found complete. Four medications and authorizations, seven children's records, seven staff's records, and two applicants' background checks were reviewed during today's inspection.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on a review of seven staff records on 06/19/2019, the center did not ensure that one staff submitted documentation of a negative tuberculosis (TB) screening within the required time frame. Evidence: 1) The record for Staff #2, hired on 03/12/2019, contained the results of a TB screening that were dated 04/10/2019. Documentation of the screening shall be submitted no later than 21 days after employment or volunteering and shall have been completed within 12 months prior to or 21 days after employing or volunteering.

Plan of Correction: Per the Center: For future hires, we will check and make sure the TB test results are submitted within the required 21 days.

Standard #: 22VAC40-185-280-B
Description: Based on observations at the center on 06/19/2019, the center did not ensure that hazardous substances shall be kept in a locked place using a safe locking method that prevents access by children. Evidence: 1) During the inspection, the licensing inspector observed a can of shaving cream in an unlocked cabinet in the Threes Room. 2) During the inspection, the licensing inspector observed a bottle of bleach water solution in an unlocked cabinet in the Threes Room.

Plan of Correction: Corrected on site. Per the Center: In the future, we will ensure these items are always kept in a locked cabinet.

Standard #: 22VAC40-191-60-B
Description: Based on a review of seven staff records and interview on 06/19/2019, the center did not ensure that one staff had a completed sworn statement or affirmation prior to the beginning of employment. Evidence: 1) The record for Staff #7, hired on 09/04/2018, contained a sworn statement dated 09/05/2018. 2) During interview, a member of management verified the sworn statement for Staff #7 was completed after the first day of employment on 09/05/2018.

Plan of Correction: Per the Center: For future hires, we will make sure the sworn disclosure statement is completed before they start.
