Comments:
An investigation was initiated on June 7, 2019 from 9:30 a.m. - 11:00 a.m. with law enforcement and local agency to investigate a self reported incident regarding care, protection, and guidance. Interviews were conducted, documentation, video footage, staff records, and children's records were reviewed. The results of the self reported incident regarding care, protection, and guidance were determined to be valid.

Violations:
Standard #: 22VAC40-185-340-A
Description: Based on interview with staff and observation of video documentation it was determined that staff did not always ensure children's care, protection, and guidance. Evidence: On 5/31/2019 at 3:51 p.m. in the two year old classroom Child A (d.o.b. 11/09/2016) is seen walking into the block area towards Staff A who is seated on the floor. Child A begins to disrupt some of the blocks in the block area. Staff A attempted to remove the child from the block area and is seen holding the hand of child A, pulling the child up to a standing position by the arm and guiding them out of the block area by the arm. Child A is then seen holding their arm and crying. Child A later goes back to staff A for comfort. After staff A notices that the child is continuing to display signs of discomfort, management and the parents were notified. Child A was later diagnosed with a dislocated elbow which is also known as "nursemaid's elbow."

Plan of Correction: Staff were retrained on how to appropriately pick up and handle children between the ages of 1 - 4 years old. A handout was provided to staff regarding nurse maid's elbow.
