An investigation was initiated on June 10, 2019 from 10:00 a.m. - 10:45 a.m. to investigate a complaint regarding a forbidden action. Interviews were conducted, documentation, and staff records were reviewed. The complaint regarding a forbidden action was determined to be valid.

Standard #: 22VAC40-185-410-1 Complaint related: Yes Description: Based on review of documentation and staff interviews it was determined that staff engaged in a forbidden action. Evidence: It was reported through documentation and staff interviews that on June 5, 2019 at around 5:30 p.m. Child A (d.o.b 06/17/2010) was sitting in the reading center. Staff A requested that all children line up at the door. When Child A did not join the group to line up Staff A grabbed the child by the upper arm roughly and pushed the child in the direction of the door. Child A reported to staff B that their arm hurt as well as the incident that took place. Staff B immediately notified management of the incident. Plan of Correction: Staff A was terminated. Management was trained on how to coach staff on appropriate interactions with children.
