The annual file review was conducted for Community Centers, and Preschool programs.
The following inspection took place between the hours of 8:30 am through 11:30 pm. Staff files were reviewed by representatives from the Commonwealth of Virginia, Department of Social Services, Division of Licensing, Fairfax Licensing Office and through the County of Loudoun, Department of Parks, Recreation, and Community Services? main office located in Leesburg, Virginia. Standard #: 22VAC40-185-160-C Description: Based on review, 1 of 17 staff files reviewed indicated that it has been more than two years from the date of the most current tuberculosis (TB) documentation on file. Evidence - 1. On the date of inspection (06/20/2019), the most current TB documentation on file for Staff #14 was dated - 1/25/19. 2. Administrative staff surmised that this documentation expired due to a clerical error in which the date was entered incorrectly into their computer as 12/5/19. Plan of Correction: Staff #14 shall be alerted and this documentation shall be obtained.
