Technical Assistance:
We discussed the requirements for diapering procedures, formula records, hand washing, and written authorizations and agreements with parents. The risk assessments for violations were provided. We talked about age appropriate sensory experiences for toddlers. A shaving cream table activity is not age appropriate for toddlers as it is difficult to prevent consumption of the shaving cream by mouth. We reviewed the requirements for documenting injuries and acceptable tuberculosis statements.

Comments:
Thank you for your assistance during today's unannounced monitoring inspection conducted from 9:11 AM to 5:15 PM. Today, there were one-hundred and fifteen children, ages infancy through school age, with nineteen staff. I viewed program activities, staffing, supervision, interactions with children, equipment, infant care and records, diapering procedures, posted information, emergency supplies and drill records, injury records, menu, nutrition, transportation procedures, medication, ten records for children, eleven staff records, staff qualifications, staff training, insurance, agency inspections, daily routines, indoor and outdoor areas. The children have enjoyed regular outdoor experiences in age appropriate playground areas. Some children have been involved in gardening. The staff were observed providing attentive supervision and frequent interactions with the children in their care. A variety of planned group activity and classroom areas for individual choices for play have been provided. Let me know if you need any assistance. 540/430-9259

Violations:
Standard #: 22VAC40-185-90--A
Description: Based on a review of ten records for children, and an interview with administrative staff, the center failed to obtain all required written agreements between the parent and center for one child's record by the first day of attendance. Evidence: Child 2 began attending on February 25, 2019. The parent's signature was not included in the agreement section with the statements that a center will notify the parent when the child becomes ill and that the parent will arrange to have the child picked up as soon as possible if so requested by the center; and that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease, as defined by the State Board of Health, except for life threatening diseases which must be reported immediately.

Plan of Correction: The director or assistant director will obtain the parent's signature for all written agreements. The administrative staff who accept enrollment forms submitted by parents will make sure all required information, including signatures, have been included before the records are filed.

Standard #: 22VAC40-185-500-A
Description: Based on observation of diaper changes of two year old children in the back bathroom area, and interviews with staff, the staff member did not wash hands after each individual diaper change of the children. Evidence: The staff member was observed removing wet diapers (some were pull-up style diapers) and checking a child with a gloved hand to determine a need for a diaper change while assisting children with toileting, and the staff member did not go to the sink to wash hands with soap and running water after each individual assistance with a child.

Plan of Correction: The director immediately talked with the staff member about hand washing requirements and required diapering procedures for the purpose of preventing the spread of illness. The administrative staff will review diapering procedures with all staff. The administrative staff will schedule regular times of observing diapering practices in each age group to make sure staff understand required diapering practices.

Standard #: 22VAC40-185-500-B
Description: Based on observation of diaper changes in the back bathroom area, and an interview with staff, all required procedures for the prevention of the spread of illness were not followed after each removal of diapers and gloves. Evidence: While conducting diaper changes (some were pull-up style diapers) of two year old children, the staff member was observed hand lifting the lid of the disposal container and not using the foot pedal mechanism for opening the lid so that there was not any hand contact with the disposal container surface when a wet diaper and used gloves were disposed. Also, used diapers were seen placed on the bathroom floor for two separate children while the staff member assisted those children and before the diapers were disposed. The bathroom floor was not a diapering surface that was cleaned and sanitized after use as a diapering surface.

Plan of Correction: The director immediately instructed the staff member to dispose of each diaper, gloves and any items used with diaper changes immediately into the disposal container by use of the foot pedal so that the staff member's hands do not have contact with the surface of the disposal container. Also, any surface, including floors, that has potential contact with body fluids will be cleaned and sanitized by staff. The floor will not be used a diapering surface. The administrative staff will review the required diapering procedures with all staff. The administrative staff will schedule regular times of observing diapering practices in each age group to make sure staff understand required diapering practices.

Standard #: 22VAC40-185-570-C
Description: Based on a review of records for infants on formula, and interviews with staff, the center failed to maintain written information from parents about the brand of formula, schedule and amount of formula for the feeding of one infant. Evidence: Staff indicated that infant 1F who started at the center while consuming breast milk began consuming formula at age seven months and the record was not updated to include the required information listing the brand, schedule, and amounts for bottle feeding.

Plan of Correction: An infant teacher will obtain the information from parents and make sure it is added to the feeding schedule form kept in the infant room. The infant staff will make sure that when formula has been added to the diet of infants that the record is updated immediately.

Standard #: 22VAC40-185-580-L
Description: Based on a review of ten records for children, and interviews with administrative staff, the center failed to obtain parental permission for transportation provided daily for one child. Evidence: Child 1 began attending on April 22, 2019 and the authorization form for transportation by center staff was not in the record. Administrative staff confirmed that child 1 has been transported to and from a local elementary school.

Plan of Correction: The director or assistant will arrange for a parent of child 1 to sign a transportation form. The administrative staff who provide registration forms to parents enrolling new children or beginning school attendance will make sure the authorization forms for transportation to and from school are included in the information obtained from parents of school age children.
